REGISTER WITH THE ALLIANCE:

Registering with the ALMT will help us not only know how much you are able to help, but will help with one of the most crucial parts of our effort. By registering you will be added to our database of supporters which we will categorize by your Legislative district. Then when our bill is wending its way through the various Legislative Committees on it way to "the floor" of the House or the Senate, we can call or email you to let you know when YOUR representatives in St. Paul will be hearing about our bill. We will let you know when it's time to "Call St. Paul" in an organized, targeted way, and be able to get our bill out of committee and out to the floor for a vote. We will even have quick on-line tutorial for how to let your rep's know where you stand so they hear your opinion in the most effective way. So even if you don't have time to work on our committees, registering and being wiling to let your voice be heard at the right moment will be a tremendous boost to the effort.

Three ways to register (Please choose one - If you have already registered on paper, you don't need to do anything else)

  • ALMT SUPPORTER EZ-REGISTRATION FORM - FOR ANYONE
    • First Step: Go to the MN State Legislature page and figure out what legislative District you live in. That page is linked RIGHT HERE. By typing in your home street address, you will be given your State Representative and State Senate district numbers.
    • Second Step: Use THIS LINK to go to our EZ-Registration form and enter in your information. Please enter in the district number for both State Representative and State Senator, not the name of the legislator.
    • If you have any questions or trouble with either link, drop us an email.

 

  • SUPPORTER REGISTRATION OPTIONS - For Massage Therapists/Students
    • PRINT AND MAIL OPTION
      • MASSAGE THERAPIST SUPPORTER REGISTRATION FORM - Open this Word Document  print and fill it out, and then mail in to ALMT PO Box 580301   Minneapolis, MN  55458-0301
    • ONLINE OPTION
      • MASSAGE THERAPIST SUPPORTER REGISTRATION ONLINE FORM - You can register with the Alliance online! Follow this link - When you finish the form and click submit, the form is sent to the Alliance Volunteer Coordinator, and you will be brought back to this website.

  • SUPPORTER REGISTRATION OPTIONS - For Non-Massage Therapists
    •  PRINT AND MAIL OPTION
      • Open this pdf form and use this form for employers, colleagues, and even clients (lay it out on the desk at your practice and invite clients to sign on). They will only be contacted to let them know when it is the perfect time to "Call St. Paul!" Make sure to tell them their information will be kept strictly confidential and will only be used to categorize their contact info by their legislative district. And just like for supporters who are Massage Therapists, contacting the legislator representing where they live as well as where they work carries a huge amount of weight. The supporter can mail it in to the address on the form, or if a Massage Therapist is collecting forms, a bunch can be mailed in at once. Or to save on postage, the info from the form can be entereed into the Online form at the link below.
    • ONLINE OPTION
      • Non-Massage Therapists can access the quick online registration form at THIS LINK. (The form of Non-MTs is shorter than the one for MTs. That is the only difference.)

This will open a Word Document that you can then edit to customize it to your specific practice, save and print. This template reflects changes in what is required to be in the CAP Client Bill of Rights, and has been checked over by the Office of Unlicensed Complementary and Alternative Health Care Practices in the MN State Department of Health, (the people who would enforce us not having the correct information).
